We are excited to have you at Optimal Care and Wellness! Below are a few things to consider before your first appointment.
Please Bring with you:
1. Health insurance card
2. Valid photo ID
3. New patient forms (see below)
4. All current medications, including eye drops, inhalers, ointments, and vitamins.
5. Payment method to collect your copay the day of the appointment. We accept credit/debit cards and cash.
