- You can stop by the office to pick up the "New patient" forms or Download the forms down below, fill out, and drop off at the office.
- Walk in, we have walk-in visits available for "sick visits".
First time visit
We are excited to have you at Optimal Care and Wellness. Below are a few things to consider before your first appointment.
Please Bring with you:
1. Health insurance card
2. Valid photo ID
3. New patient forms (see below)
4. All current medications, including eye drops, inhalers, ointments, and vitamins
5. Payment method to collect your copay. We accept credit/debit cards and cash.
Self-Pay
No insurance? No Problem
- Optimal Care and Wellness offers a self-pay discount if you are uninsured.
- Our goal is to deliver quality care that is affordable to our community regardless of insurance status.
- Office visit starts at $85. Payment at time of service is required.
